Steps:

1. Obtain the application form:
   • Visit the Directorate of Social Welfare, Port Blair, or the sub-divisional offices of Child Development Project Officers (CDPOs).
   • Alternatively, download the form from the official website.
2. Submit the filled application form:
   • Complete the application form with all required details.
   • Attach all relevant documents.
   • Submit the completed form to the offices of the respective Child Development Project Officer (CDPO).

Notes:

• Note 01: Notify the Director of Social Welfare promptly about any changes in address or employment status within 15 days.
• Note 02: Failure to inform the Director of Social Welfare within the stipulated 15-day period may result in appropriate action.
