Each disabled person will receive a maximum of Rs. 6,00,000

Surgical Grant For The Prevention Of Disability Uttar Pradesh : Eligibility

• Annual income of the applicant or their guardian must not exceed Rs. 60,000.
• Applicant must be an Indian citizen.
• Applicant should be a permanent resident of Uttar Pradesh or have lived in the state for at least 5 years.
• Applicant must not have any criminal record.

Surgical Grant For The Prevention Of Disability Uttar Pradesh: Offline Application Process

1. Download the Application Form:
   • Visit page 18 of the website to download the application form.
2. Fill Out the Application Form:
   • Complete all sections of the application form accurately.
3. Attach the Following Documents:
   • (a) Certificate of disability issued by a qualified medical authority.
   • (b) Certificate of annual income from all sources.
   • (c) Estimated expenditure details for the surgery provided by the superintendent or in-charge of the State Hospital.
4. Submit the Application:
   • Submit the completed application form to the District Disabled Welfare Officer.
   • Ensure the application includes a recommendation letter.
   • The District Officer will forward the application and recommendation to the Director.
5. Emergency Applications:
   • In cases of emergency, applications can be directly submitted to the Director.
